In order for us to serve you, we will need you to complete and send to us confidential information about you or your child. This information allows us to understand vital history and influences impacting communication or learning in order to develop appropriate testing or goals. All persons are asked to complete Form # 1 or the Client Information Sheet followed by the appropriate age-level Case History form # 2. Next, the Interest Inventory # 4, 5 gives us insights into students’ likes and dislikes which is important in assembling effective materials for young children. If you would like us to consult with other specialists, teachers or employers, we will need your written permission provided by the Release Form # 6. Finally, should you have testing conducted within the last two years that you would like us to review, you may send these records to us.
